- Your contact information: Include your name, address, phone number, and email address.
- A statement of good faith: Confirm under penalty of perjury that you have a good-faith belief that the material was removed or disabled due to a mistake or misidentification.
- Consent to jurisdiction: Agree to the jurisdiction of the federal court in your area or, if outside the United States, the jurisdiction of the courts in the country where iPagal operates.
